Live Risk Matrix: Reducing
Road Safety Risks reduced from High (15) to Low (4) after positive keywork, clearly shown through the regular updates and evidenced in the risk over time dashboard insight.
How traditional care platforms capture the care, but bury the outcomes.
Hereβs what happens when record keeping becomes an administrative tickbox and not the solution.
Disconnected Static Records
Care plans, daily logs, and risk assessments are trapped in generic file storage platforms and paper binders. Time spent duplicating information across several data entry points increases stress and error likelihood, whilst leaving little room for spending quality time with the children.
Hidden Progress
Real developmental breakthroughs and small daily wins get buried inside dense operational systems. Without live trajectory tracking, evidencing genuine distance travelled becomes an uphill struggle.
Untracked Risks
Static risk assessments quickly become obsolete as young people's behaviors and dynamics change. Missing real-time updates and staff sign-offs leaves homes exposed to unmanaged risks and compliance vulnerabilities.
Compliant by design, child-centred by nature
Leoβs Journey
Leo Randle
Current Situation and Needs
Leo has not attended school for five months due to multiple placement changes. As a result, Leo's anxieties about attending school due to historical bullying have resurfaced, and he has absconded from his new school on two occasions within minutes of arrival.
What do we want to achieve?
Leo would benefit from support to develop his self-esteem so that he feels more confident in a large group setting. This will enable him to navigate the challenges that he could face in school amongst peers, and increase his feelings of safety. Building relationships with school staff will also ensure that Leo has a safe adult to talk to should he feel anxious or concerned at school.
How will we achieve this?
- Predictable morning routine.
- Staff accompaniment to the school reception area with agreed safe-exit plan.
- Regular Keywork sessions, focusing on self-esteem and well-being.
- Leo to take agreed fidget toys into school to support with self-regulation.
- Pre-term meeting with school staff to provide historical context and explore option of a buddy system.
How will we know if it has been achieved?
